Why it matters

Coverage changes between visits

A Medicaid patient switches MCOs while remaining eligible. A Medicare Advantage plan changes. Employer coverage ends. Primary and secondary payer information no longer matches. For Medicaid, changes in laws, state rules, or patient circumstances can mean new renewal steps, and missed notices can put eligible patients at risk of losing coverage.

Medicaid/Medicare Coverage Monitoring Services give your organization an ongoing, coordinated way to catch these gaps earlier and help patients act on them, with your team in control of every outreach decision.

What the service includes

Monitoring, outreach, and follow-through in one service

01

Ongoing automated coverage monitoring

Recurring checks set with your team help identify Medicaid eligibility and managed care organization (MCO) changes, Medicare Advantage changes, commercial plan changes, and primary/secondary payer changes.

02

Coverage loss and risk identification

Patients whose coverage has lapsed, is approaching renewal, or shows signs of risk are flagged and grouped so your staff knows who needs attention first.

03

Provider coordination

Outreach plans, scripts, and patient lists are reviewed and approved by your organization. Nothing goes to a patient without your sign-off.

04

Patient outreach

Our team contacts patients through provider-approved channels to explain what may be needed and help them take the next step toward keeping their coverage.

05

Medicaid portal-update assistance

With the patient’s consent, we help Medicaid patients review and update household, contact, and income details as applicable on government portals.

06

Medicaid renewal assistance

Medicaid patients get guided help understanding renewal notices, gathering requested documents, and completing renewal steps when they may be eligible.

How it works

A coordinated workflow, led by your organization

  1. 01

    Align with your team

We agree on patient populations, outreach channels, approved messaging, and how and when your staff wants to be updated.

  2. 02

    Monitor and identify

Checks run on the schedule we set together. Medicaid termination, reinstatement, MCO switches, enrollment and effective-date mismatches, Medicare changes, and payer-order discrepancies are flagged for review.

  3. 03

    Consent and outreach

We reach patients through approved channels and obtain the consent or authorization required before assisting with their information.

  4. 04

    Coordinate the next steps

Your team reviews coverage and payer discrepancies with patients. For Medicaid, we also help with accurate portal updates and renewal documentation through the appropriate state process.

  5. 05

    Follow up and report

We track open items, follow up with patients, and keep your team informed of status so front desk and billing staff can plan ahead.

How we work with patients

Consent, accuracy, and your oversight

Patient consent comes first

We assist patients only with their consent or authorization, following the requirements your organization and the applicable state set.

Provider-approved outreach only

Your organization controls who is contacted, how, and with what message. Our team works within those boundaries.

Accuracy over outcome

The goal is correct, current information. We never encourage patients to report anything other than their true circumstances.

Visibility for your team

You see who was contacted, what was completed, and what remains open, so coverage questions do not catch staff off guard at check-in.

Fewer coverage surprises at check-in

Medicaid/Medicare Coverage Monitoring Services FAQ

Common questions

What are Medicaid/Medicare Coverage Monitoring Services?
The service includes Medicaid Eligibility Monitoring, Medicaid MCO Monitoring, Medicare Advantage Monitoring, Coordination of Benefits Monitoring, and Commercial Coverage Monitoring, with provider-coordinated follow-up. Network & Authorization Monitoring is planned for Phase 2, not a current capability. Medicaid support also includes consent-based assistance with accurate government-portal updates and renewal steps.
How is Medicaid MCO Monitoring different from eligibility monitoring?
Eligibility monitoring tracks Medicaid coverage termination, renewal gaps, and reinstatement. MCO monitoring tracks changes in a Medicaid managed care organization, including MCO switches, enrollment changes, and effective-date mismatches. A patient can remain Medicaid-eligible while the managed care plan responsible for their coverage changes.
What does Medicare Advantage Monitoring track?
The Medicare Advantage capability monitors MA plan changes, disenrollment, and coverage transitions. Broader Medicare monitoring also includes Part B status and payer changes. Monitoring does not enroll patients in a plan or change their Medicare coverage.
Is Network & Authorization Monitoring available now?
No. This capability is planned for Phase 2. Its planned scope is network participation or authorization impacts following plan changes. It should not be relied on as a current monitoring capability.
What does Commercial Coverage Monitoring track?
The commercial module monitors employer insurance changes, plan terminations, and benefits to help your team identify changes that may affect upcoming care and billing.
What does Coordination of Benefits Monitoring track?
This module monitors primary and secondary payer changes and discrepancies. It flags potential payer-order issues for your team to review and verify with patients and payers rather than assuming coverage or payment is guaranteed.
Does this service guarantee that a patient keeps Medicaid coverage?
No. State Medicaid agencies make all final eligibility determinations. Updating information or completing renewal steps does not guarantee coverage. Our role is to help eligible patients avoid losing coverage because of missed steps or outdated information.
How often is coverage monitored?
Monitoring frequency is set with each provider organization based on its patient population, selected modules, workflows, and available payer information. Medicaid rules vary by state, so there is no single cadence that fits everyone.
Do you contact patients without the provider knowing?
No. Outreach is coordinated with and approved by the provider organization, including which patients are contacted, the channels used, and the messaging.
What patient information do you help update?
For Medicaid, with patient consent or authorization, we help patients review and update household, contact, and income information as applicable, and assist with renewal forms and requested documentation through the appropriate state process. Other modules focus on monitoring and provider-coordinated review of coverage and payer information.
How does my team stay informed?
Your team receives ongoing visibility into which patients were flagged, who was contacted, what steps were completed, and which items still need follow-up.
